How to reproduce: In artifact view create a new artifact from the artifact link field

In your possible tracker selection make sure that an artifact id is matching part of an other (example I have a tracker a with id 1, and a tracker with id 11)

Switch from tracker, each time you'll select a tracker, the form won't be loaded dynamically
